What shipped

• PR #3639 merged: fix for the rebase-conflict re-route bug from yesterday. Dispatch prep no longer aborts the in-progress rebase that auto_merge deliberately preserved, and the re-dispatch prompt now tells the agent about the conflict. Root-caused in issue #3638, which was closed today.
• 14 tasks marked done across managed projects in the last 24h. 16 PRs created, 34 pushes, 17 review starts, 16 review decisions. Throughput recovered fully after the 2026-09-28 to 2026-10-02 idle stretch.

What failed

• One minimax/opus run failed with API Error: 402 insufficient balance (1008); failover to claude worked and the task completed. Generic cooldown path as designed. minimax and its models remain in 1-day cooldowns.
• One review run timed out at 08:37 UTC (run_type=review). The engine retried (attempt 2) 13 seconds later, the review approved, and the PR merged. Self-recovered, no action needed.
• One opencode/opencode-mimo-v2.6-flash-free run timed out. Single event, no pattern.

Operational health

Task runs in the 24h window:

task_activity: 111 status changes, 37 dispatches, 34 pushes, 28 branch deletes, 17 routed, 17 review starts, 16 review decisions, 16 PR creates, plus one each of timeout, reroute, and error (all covered above).

/opt/homebrew/var/log/orch.error.log is 0 bytes. Brief GitHub transport errors appeared at 22:02 UTC (network hiccup, requests retried, circuit-breaker not tripped).

Active cooldowns: codex (persisted, ~10d), minimax agent-wide plus haiku and opus model cooldowns (1d each, from the 402).

Stuck tasks

• internal:174453 (macro monitor daily job): still blocked with merge conflict retry limit (3) reached. This is the task that exposed yesterday's bug. The fix is merged on main, but the task itself needs the operator to unblock it (orch task retry or equivalent) so it re-dispatches through the fixed path. Its PR #3319 is still open and conflicted.
• internal:154443 (security audit): blocked, long-standing, awaiting human.

Routing accuracy

Routing matches prompts/skills/orch/SKILL.md and settled policy. The 402 on minimax produced a proper failover and cooldown; the review timeout retried without intervention. No silent model failures, no agents stuck in unexpected cooldowns. The codex persisted cooldown dates from an earlier failure window and expires on its own.

Priorities for tomorrow

1. Operator: unblock internal:174453 so the daily macro monitor job resumes through the fixed rebase-conflict path.
2. Operator: top up the minimax balance or let the cooldown expire; the generic backoff handles it either way.
3. Watch that PR #3319 merges cleanly after the unblock.

No GitHub issues filed. The one real root cause found yesterday shipped its fix today; everything else was expected generic behavior.
