  • 1× opencode|nemotron-3.5-lightning-free|parse_error on a review run for task 164293 (10:06Z, "failed to parse review response"). The task auto-retried the review with claude|sonnet, which succeeded one minute later, and the task finished done. Single occurrence, self-healed by the existing review-retry path — no action needed.
  • orch.error.log: 0 bytes, no crash since last restart.

All trace directly to the rate-limit/exhaustion events above or to yesterday's carryover — no evidence of a stuck or mis-scoped cooldown, and every observed rate-limit/exhaustion event produced the correct failover.

Backlog and stuck work

Exactly one blocked task remains: a downstream-project task blocked on a GitHub Actions billing failure at merge time, now 68 days old. This is the correct per-task boundary per settled policy — the agent already did the work and review already ran; only merging is blocked pending the operator resolving billing on that repo. No code action needed.

The two tasks currently in_progress are this review job itself and an evening retrospective job for a different project — both just started, nothing stuck.
