It has not — and the reason turned out to be a silent failure, not just elapsed cooldown. One issue filed: #3561.

Routing and cooldowns
No repeated-cooldown or silent-model-failure patterns beyond the two self-healed runs above. No new circuit-breaker trips. The cooldown:github:5xx KV entry is stale (expired 2026-08-17) — not a factor in anything below.
Backlog and stuck work — the real story of today
Only two tasks are blocked repo-wide right now: a GitHub Actions billing block on another project (policy-expected, no action needed) and 156854 (issue #3535, PR #3536), the same task flagged in the last several daily reviews.
Yesterday's priority #1 was to confirm whether #3559 — the "give the PR an active chance to recover" fix that calls update_pr_branch to merge base into the PR branch and re-trigger CI — actually unstuck this PR once its 24h cooldown elapsed. It did get a cooldown-eligible sweep cycle today at 04:54:43Z, over 24h after #3559 shipped. But PR #3536 is unchanged: head_sha and updated_at are identical to when the PR was opened on 2026-08-20, and mergeStateStatus is still BEHIND. auto_unblock_count advanced from 9 to 10 as if the sweep made progress, but the underlying update_pr_branch call has evidently never actually landed a change.
Tracing why: both the success and failure branches of the update_pr_branch/enable_auto_merge calls in try_unblock_ci_failure_task log only at debug level — below the default production log level. So if the update-branch call is failing (permission, branch-protection, transient API error, whatever), nothing surfaces it anywhere; the sweep just keeps incrementing the counter every ~24h forever, looking identical in the logs to a working recovery. Filed #3561 to diagnose the actual failure and get it logged at a visible level — the point of #3559 was to make this recovery active, and right now it's silently behaving like the old passive poll it replaced.
One separate, lower-priority observation while investigating: a different long-stuck task from the same family (an earlier daily-review run whose PR had gone through this same CI-failure-blocked cycle) shows its content already present on main via a separately-landed commit, while its own originating PR remained open. Its task row was closed out with a written summary but with no corresponding task_runs/task_activity trail — consistent with a one-off manual cleanup outside the normal engine flow rather than a code path worth a bug report. Flagging only for visibility; no action needed unless the pattern recurs through the normal automated flow.
Policy alignment
Current operational behavior matches prompts/skills/orch/SKILL.md — no drift detected.
