Daily Review — 2026-08-25

The headline: second quiet day in a row — one silence-detection false-kill bug found and fixed same-day, nothing else new

Window: 2026-08-24T23:01Z → 2026-08-25T23:03Z. Two commits landed, one issue closed (fixed same-day it was filed), no crashes, no new failure patterns. The two long-stuck PRs from last week remain stuck, unchanged.

CommitIssueSummary
531bc51f#3554 → PR #3555Fixed a false-positive silence-detection kill: the agent runner piped {agent_cmd} | tee into the tmux pane, and non-TTY stdout made several CLI runtimes switch to full block-buffering, so nothing reached the pane for minutes even while the agent was actively streaming and making API calls. Silence detection (10-min grace period) then killed the live session, applied a full model cooldown, and re-routed the task, discarding real in-progress work. 14 such kills over 7 days (11/14 on claude/sonnet, an 8.4% false-kill rate on that agent), all self-healed on retry but wasting 3-4h of engine/agent time and polluting cooldown signal with false model-health data. Fix: run the agent under a real pty (script -q /dev/null-style) instead of a plain pipe, so tee's write timing no longer depends on the child's non-TTY buffering heuristic.

Closed today: #3554 — filed and fixed same-day.

Still open, unchanged:

  • #3535 — opencode "not available in your country" misclassification. Fix already committed (ae8146bb); PR #3536 still blocked, same as below.
  • #3453 — pending-CI-status prose causing review parse errors, 27 days old, no activity since filing. Awaiting the next full issue-ingest rescan to pick it back up (per #3545's already-shipped fix); nothing further to evaluate per deployment policy.

The 3 claude sonnet failed rows are the silence-detection false kills that #3554/531bc51f fixes:

  • internal:157281 (21:06:44Z, 08-24) and internal:157332 (08:01:09Z, 08-25) — both silence detection set task to new/routed, both self-healed on immediate retry, both predate the fix (landed 22:01:49Z on 08-25). This is expected: the fix can't retroactively cover runs that already happened. No post-fix recurrence observed yet — nothing to evaluate until the next window, per deployment policy.
  • kimi opus rate_limit x1 — same already-understood billing-cycle-exhaustion pattern #3551/490543fe fixed yesterday, this is a stale row from before that fix (task self-healed via opencode fallback).
  • opencode x-preview-f-free parse_error x1 — single occurrence, retried successfully on claude/sonnet, matches the long-documented class of opencode-review parse issues, not a new pattern.

orch log 200

No WATCHDOG, no panics, no unexpected error lines beyond the accounted-for silence-detection/rate-limit rows above. recent_rate_limit_counts cooldown-health check running cleanly every tick.

Routing and cooldowns

No repeated-cooldown or silent-model-failure patterns. No evidence of the stuck-task reclaim race (#3518 → #3523 → #3526) recurring. No new circuit-breaker trips.

Backlog and stuck work

  • internal:156996 (PR #3538) and 3535 (PR #3536) remain blocked, now 5 days (age: 4d/5d in the queue) on the same CI-failure-limit-reached state, confirmed still mergeStateStatus: BEHIND / mergeable: MERGEABLE against current main. Both PRs opened 2026-08-20, only 1 commit each — they never got a chance to catch up before hitting the retry cap. Unchanged across 5+ consecutive reviews now; this is a designed safety valve (auto-merge intentionally stops retrying once ci_merge_failures hits the cap so a flaky rebase loop can't spin forever) — resolving it is an operator rebase-and-retry, not a code fix, since the underlying issue in each PR is already fixed on HEAD.
  • Other blocked tasks in the global queue are all previously-diagnosed, policy-expected states: GitHub Actions billing failures (block at merge time, correct per-task behavior), review-rebroadcast escalation, and max-review-cycle blocks. No new shapes.

Policy alignment

Current operational behavior matches prompts/skills/orch/SKILL.md — no drift detected.

Issues Filed Today

None. #3554 was filed and fixed within the window; nothing new met the bar for filing — no unexplained crashes, no new failure shapes, and the two stuck PRs are a known, already-flagged, operator-action item rather than a fresh bug.

Priorities for Tomorrow

  1. PRs #3536 and #3538 still need an operator rebase-and-retry — both remain BEHIND main, unchanged across 5+ consecutive reviews now. Both PRs' underlying fixes are already on HEAD; only the branch needs to catch up.
  2. Confirm #3554's pty fix holds — watch for any further silence detection set task to new/routed rows dated after 2026-08-25T22:01:49Z; none observed yet since the fix landed only ~1h before this review's window closed.
  3. Confirm #3453 gets picked up by the next full issue-ingest rescan — no action needed until then.
