The headline: the self-improvement loop worked end-to-end again, faster than yesterday — 24 minutes from filed to merged

Task internal:156798 (a scheduled self-improvement/debug-agent job) reviewed the last 12-24h of failures and blocked tasks, correctly recognized that two failures (a Kimi billing-cycle event, a Codex gpt-5.4 failure) were already covered by existing fixes and generic cooldown, and isolated one genuine structural gap: auto_recover_rebroadcast_blocked_tasks() — the only function that clears the review agent rebroadcast escalated after repeated retries block reason — only runs from per-active-repo sync_tick, while the sweep that sets that same block reason (refire_and_escalate_stale_needs_review_global) is DB-wide. Any task belonging to a repo later removed from the active project list gets escalated into this block state but can never be recovered from it. It filed #3532 at 21:07Z with exact evidence (three stranded tasks, one with an already-merged PR that would never reconcile to done), and a follow-up task merged the fix (619799c4) at 21:39Z, 32 minutes later.

619799c4#3532Added auto_recover_rebroadcast_blocked_tasks_global(), wired into the same global-sweep site as the escalation path (tick_recover_stuck_tasks in src/engine/tick.rs), so recovery now reaches tasks in any repo regardless of active-project status. Also added the PR-state check the old function lacked: checks the attached PR's live state before resetting to NeedsReview — merged PRs mark the task done directly, closed-unmerged PRs stay blocked instead of re-dispatching a review agent against a PR that no longer exists. Handles edge cases (missing PR, non-matching block reasons, same-tick freshness guard, no-routable-agent guard). Full 3924-test suite, cargo fmt, cargo clippy all clean.

Closed today: #3532 (fixed same-window, 32 minutes filed-to-merged).

Still open: #3453 (bug(review-prompt): pending CI status prose still causes review parse errors), now 21 days old, no new occurrence today — the only open issue in this repo.

Note on #3532's fix: the three tasks that motivated the issue (external ids 458, 490, 493) are still blocked in the live DB as of this review, because the fix exists only on HEAD — expected, not an operational problem (see repo policy on deployment lag). Worth checking again once the running service picks up the change: task 458's attached PR (#462) merged back on 2026-08-12 and should reconcile straight to done.

Nothing here rises to a pattern: the claude/sonnet failed/null-outcome rows and the single codex gpt-5.4 failed are one-off events already covered by generic retry/failover, not a recurring classifier gap. The single kimi/opus rate_limit row is the expected shape now that #3529's billing-cycle-vs-rate-limit fix is live — one real rate limit, correctly classified, no misclassification evidence this window.

Stuck-task reclaim race (#3518 → #3523 → #3526): quiet for a second day

No reclaiming early / dispatch-guard-race log lines this window. Two quiet days in a row now, though the last "quiet gap" before #3526's recurrence also lasted 16h before repeating — still too early to call it resolved. The info-level dispatch-guard logging from 9e098c9b remains live and ready to capture direct evidence if it recurs.

Tick performance: 1 slow tick, no stalls

One slow-tick warning today (50.1s, 16:01:04Z, coincident with a router LLM pool timeout for kimi:haiku that correctly recorded a cooldown and retried next tick — expected backpressure, not a bug). No watchdog stalls, no circuit-breaker trips. A handful of GitHub transport-level connection failures (16:40Z, 18:28Z, 18:47Z, 22:35Z) correctly did not trip the github:5xx circuit breaker — transport-vs-5xx classification from #3492 continues to hold.

Routing: cooled-agent LLM proposals continue to surface, safety net still catching them

4 occurrences of LLM selected cooled agent/model; rerouting to available agent today, all minimax falling back to claude. Same shape as every prior day, zero functional impact, expected per repo policy (pre-emptive routability checks reduce but don't eliminate these; the sanity-check fallback is the designed backstop).

Backlog and stuck work

Unchanged in shape from yesterday: several GitHub Actions billing failure blocks at merge time in the bean project (correct per-task policy, operator-controlled), a handful of long-idle bean/oblivion items (10–138 days) with no new activity, already diagnosed in prior reviews as operator-controlled or config-scoped state. The three rebroadcast-block tasks covered by #3532 above are the only state change worth tracking — watch for them to clear once the fix is live.

None from this review. internal:156798's debug-agent task already filed the one substantive finding of the window (#3532, fixed same-window) — no additional operational problems met the bar: the stuck-task reclaim race has no new occurrence to add evidence for, the GitHub transport blips are external and handled correctly, and the cooled-agent reroutes are the existing safety net working as designed.
