The one genuinely new finding this review is a narrow opencode error-classification gap (filed as #3475), not a repeat of anything already tracked.

  • failed (codex gpt-5.4, ×2)model unavailable (gpt-5.4): Model metadata for 'gpt-5.4' not found. Already covered by codex.rs's ModelUnavailable classification with dedicated regression tests; cooled at the model level (codex:gpt-5.4 currently has 12h3m remaining). Working as designed.
  • parse_error (opencode/ling-3.0-flash-free, task 155730, 08:04 UTC) — this is one of the three exact examples cited in #3473 (opencode step_finish reason="length" truncation misread as malformed output). That fix is already in flight: task 3473 is in_review with a PR up, no new issue needed.
  • failed (opencode/ling-3.0-flash-free, task 155736, 16:06 UTC)new finding. Upstream returned HTTP 404 "This model is unavailable for free. ... use this slug instead: ..." — a permanent free-tier discontinuation, not a transient blip. The current classifier's is_model_unavailable check doesn't recognize this phrasing, and the message also matches the "upstream request failed" substring that detect_network_error treats as transient, so it fell through to a generic NetworkError instead of ModelUnavailable — meaning no model-specific cooldown was applied. Same problem class as #2228 (deprecated/no-endpoints-found phrasing), just a new upstream wording variant. Filed as #3475.
  • rate_limit (opencode/nemotron-3-ultra-free, ×1) — upstream 502 from the Nvidia-backed free tier, correctly classified, self-recovered.
  • rate_limit (minimax opus, ×1) — billing-cycle exhaustion, same recurring class as recent days. minimax:opus cooldown is now 6d22h remaining (up from 17h2m yesterday), consistent with the settled exponential escalation for repeat billing-cycle failures (24h → 7d cap). Not a bug — the backoff is doing exactly what it's designed to do given repeated hits.

Backlog and Stuck Work

  • 56 tasks blocked (unchanged from yesterday) — 44 in one downstream repo (still CI failure limit reached during auto-merge, PRs still open) and 12 in another downstream repo (billing-failure + one repeated-rebroadcast escalation). Both are the correctly-designed per-task block-at-merge-time behavior.
  • #490/#493 (needs_review, downstream repo) — still needs_review_refires = 0 and updated_at still 2026-07-20, unchanged for the third review in a row despite the repo-agnostic sweep (ea4772a9) landing two days ago. Traced the sweep code again this review: the query is genuinely global (no repo filter), the per-task skip conditions (dispatching-map entry, all-review-agents-cooled) are both transient and shouldn't persist across two full days of ticks, and the repo is confirmed commented out of the active projects: list — exactly the case the sweep targets. No bug conclusively identified from static reading alone. Carrying this forward as a watch item rather than filing on inconclusive evidence.
  • #3453 (bug(review-prompt): pending CI status prose still causes review parse errors) — still open, zero comments, now 8 days old, still no corresponding orch task. Third review flagging this; the gap is a missing task, not a missing bug report.
  • Orch's own repo queue is clean: only this review task and the retrospective task in_progress, one PR (#3473) in_review, nothing else blocked or stuck.

Filed today:

  • #3475 — opencode 404 "model is unavailable for free" misclassified as transient NetworkError instead of ModelUnavailable, so a permanently-discontinued free-tier model doesn't get its model-specific cooldown. Root-cause classifier gap, one occurrence so far, same pattern class as the already-fixed #2228.

Everything else this window — two silence detections, two codex model-unavailable cooldowns, one opencode 502, one already-in-flight parse_error (#3473) — is handled correctly by existing generic mechanisms.


Priorities for Tomorrow

  1. Re-check #490/#493 again. If needs_review_refires is still 0 on the next review, this graduates from "watch" to "needs live tracing" — the static code read didn't turn up an explanation, so the next step is adding a temporary log line or checking error events in task_activity around the sweep's call sites, not more static reading.
  2. Get #3453 a task. Third review flagging the same gap — the issue itself needs a manual task before its review-prompt bug can be worked.
  3. Watch #3475 and #3473 land. Both are narrow, well-scoped classifier fixes already diagnosed to the exact line; confirm neither recurs once merged.
